Record a quality incident

This article is about the EY Catalyst Smart Daily Management application.

1. Launch the My Tasks application on the Smart Daily Management home screen; or select the hamburger button in the top menu panel and select My Tasks.
2. Ensure that you have selected the correct asset from the context menu. Select +Incident for Quality Risk.
3. In the Record Quality Incident page, select Name and select the quality incident that matches the current incident.
4. Select Asset and select the specific asset that the incident applies to.
5. Enter a summary of the incident in the Summary box.
6. Enter a description of what happened in the Description box.
7. In the Follow-Up Activities Required box, enter a list of any follow-up actions that are required as result of the quality incident.
8. You can choose to upload media files to illustrate the quality incident:

• • Select Image, Video, or Document to upload a relevant file.
  • Using the file navigation system of your web browser, navigate to the files you want to upload, select the files, and click Open.

9. Select Save Changes.
