This article is about the EY Catalyst Smart Daily Management application.
- Launch the Breakdown Elimination module; or select the hamburger button in the top menu panel and select Daily Management > Breakdown Elimination.
- Ensure that you have selected the correct asset from the context menu.
- Select the row for the breakdown you want to edit.
- Select Analysis and Follow Up.
- Select Fix now.
- The Name field is populated automatically with the name of the user who logged the breakdown.
- In the Observed Phenomena box, enter a description of what is found during the in-depth breakdown analysis.
- In the Theory of failure and root cause box, enter a description for what caused the breakdown and the root cause identified at the end of the analysis.
- Next, provide information about the vendor related to the affected equipment. Enter the vendor details in the Name, Phone and Contact Name boxes.
- Provide a description of any permanent countermeasures taken to prevent similar breakdowns in future. You can enter two alternative suggestions in the Option 1 and Option 2 boxes.
- Specify whether the breakdown requires any new maintenance or operating procedures or changes to existing procedures:

- If the breakdown requires any new maintenance or operating procedures or changes to existing procedures, select Yes. Then enter a description of the changes required in the Maintenance or Operating Procedures: new or changes required box. Next, specify a target date by selecting the Target Date box and selecting a date using the calendar tool. Assign responsibility for this task to someone by selecting their name from the By Who drop-down list.
- Select No if neither is required.
- Specify whether the breakdown requires any new training, OPLs, or changes to existing training and OPLs:

- If the breakdown requires any new training, OPLs, or changes to existing training and OPLs, select Yes. Then enter a description of the changes required in the Training/OPLs: new or changes required box. Next, specify a target date by selecting the Target Date box and selecting a date using the calendar tool. Assign responsibility for this task to someone by selecting their name from the By Who drop-down list.
- Select No if neither is required.
- Specify whether the breakdown requires any changes to clean, inspect, lubricate (CIL) tasks or time-based maintenance tasks (TBMs) tasks:

- If the breakdown requires any new CILs, TBMs, or changes to existing CILs and TBMs, select Yes. Then enter a description of the changes required in the CILs and TBMs: new or changes required box. Next, specify a target date by selecting the Target Date box and selecting a date using the calendar tool. Assign responsibility for this task to someone by selecting their name from the By Who drop-down list.
- Select No if neither is required.
- Specify whether the breakdown requires any changes to spare parts – for example, if any spare parts need to be deleted:

- If the breakdown requires changes to spare parts, select Yes. Then enter a description of the changes required in the Spare Parts: changes in Type, Quantity, or Deletion required box. Next, specify a target date by selecting the Target Date box and selecting a date using the calendar tool. Assign responsibility for this task to someone by selecting their name from the By Who drop-down list.
- Select No if neither is required.
- Specify whether the breakdown requires any changes to engineering standards:

- If the breakdown requires any changes to engineering standards or new entries in the MP database, select Yes. Then enter a description of the changes required in the Engineering Standards: changes or MP Database input required box. Next, specify a target date by selecting the Target Date box and selecting a date using the calendar tool. Assign responsibility for this task to someone by selecting their name from the By Who drop-down list.
- Select No if neither is required.
- Specify the details of the verification plan. Enter information about what, when and who in the Verification Plan: (What, When & Who) box.
- Specify the details of the reapplication plan. Enter information about what, when and who in the Reapplication Plan: (What, When & Who) box.
- Indicate whether countermeasures are needed and planned by selecting either No or Yes. If countermeasures are needed, indicated whether countermeasures are already resolved:

- Select Yes if countermeasures are complete.
- Select No if countermeasures are still in progress.
- Select a maintenance method from the Future Maintenance Method drop-down list.
- Select Save.
